For our New Patients, please call (505) 327-4863 to schedule your initial appointment. Once your appointment has been scheduled, you will receive a Welcome Email from us with a link for the New Patient Forms.

For your convenience, we have also made our new patient forms available to you in a printable PDF format.

If you choose to print the forms, please remember to bring the completed forms with you to your first appointment. We ask that you arrive 15 minutes before your appointment time to allow us to enter your information into our system. If you have any problem getting the forms to print properly, please call our office and we will gladly send or fax them to you.

We also work with most major dental insurance providers and are happy to assist you in understanding your coverage and benefits.

Insurance Policy Information

Our knowledgeable staff is available to answer your questions and assist with claims processing.

Please bring your insurance card to your appointment, and we will submit claims on your behalf. While we accept most major dental insurance plans, it's always a good idea to verify coverage with your provider before your visit. We are happy to provide estimates for treatment costs based on your insurance, but ultimately, the patient is responsible for any balance not covered by their plan.

Patient Questionnaires & Health Forms

To ensure we provide the most personalized and effective dental care, Desert Hills Dental Care utilizes patient questionnaires to gather essential information about your health history and specific concerns. Completing these forms helps our dental team understand your unique needs, including any issues related to sleep or temporomandibular joint (TMJ) disorders.

We offer several specialized questionnaires, such as the Sleep Questionnaire and the TMD Questionnaire, which allow us to assess potential problems like sleep apnea or jaw pain.
